Purpose: This form is to be used by staff members, families, volunteers, or visitors to report any concern, disclosure, or incident involving the safety and wellbeing of a child at Regional Early Education and Development. Reports may be made anonymously; however, providing contact details may assist in follow-up and support.
Confidentiality Statement: All reports are treated confidentially and shared only with authorised personnel or statutory agencies where required by law. The safety and wellbeing of children is our highest priority.
